Plan will pay 80% up to 1st $5,000, then 100% to plan max

Maximum of 3 plan life deductibles per family
$20,000 coverage for pre-existing conditions for US citizens travelling outside US. Reduced to $5,000 for travelers aged 70+ years
$15,000 coverage for pre-existing conditions for Non US citizens under 70 years of age in US

The usage of deductible and coinsurance:
First time you visit the doctor with sudden eye pain the bill amounts to $300 for a filling
You will have to pay the $250 as the chosen deductible is $250 and the remaining amount, $50 the plan pays
A few months later, you meet with a motor accident and get hospitalized to undergo a surgery for $50,000
Now the plan will pay 80% of the first $5,000. ie. Company will pay $4,000 and you will pay $1000
The plan will then pay the remaining eligible expenses
